Care & cost guideInsurance

Will the plan pay for the care being proposed?

Ask about the exact visit, test and medicine. Keep each answer next to the name it covers.

Ask about the named medicine.

A formulary is an insurance plan’s list of the medicines it covers.

Tell your insurer the medicine’s exact name and why whoever is treating you wants to use it. Ask what you would owe, rather than asking whether the drug is covered in general.

  • Is this medicine on my plan’s list?
  • Does the reason for treatment affect the answer?
  • Which pharmacy can fill it under the plan?
  • Can the plan send its answer in writing?

Ask about the approval step.

Prior authorization means the plan asks to approve payment in advance.

Ask the care service who sends the paperwork and who tells you the result. Include any charge for that work in the cost conversation.

The next two sections concern weight-loss care. They are useful if you are also asking about weight loss. For tissue-repair care, give your insurer the medicine’s exact name and ask about coverage for its proposed use.

The Centers for Medicare & Medicaid Services (CMS) is the Medicare agency.

Evidence note

The usual Medicare drug benefit has limits.

Medicare calls its prescription-drug benefit Part D.

As of September 2026, medicines taken only for weight loss are excluded from Medicare’s usual drug benefit.

As of September 2026, Medicare may consider payment for the same medicine when used for another qualifying condition, such as diabetes. Your plan still checks its medicine list and payment approval rules.

Ask your plan whether Medicare’s rules accept the proposed reason for treatment. Ask whether your plan will pay for it.

CMS: the rule on the reason for treatment

If you are seeking weight-loss care, check this temporary programme.

The price below is for this weight-loss programme. It is not a quote for tissue-repair treatment.

GLP-1 medicines copy a hormone, a body signal that affects appetite and blood sugar.

Evidence note

The temporary programme has its own terms.

As of September 2026, a monthly supply of certain weight-care medicines costs eligible people $50 under the Medicare GLP-1 Bridge.

The programme begins on July 1, 2026; December 31, 2027, is its last day. It sits outside the usual Medicare drug benefit.

As of September 2026, the programme has rules about age, weight, other health conditions and the type of Medicare plan. It requires a prescription and payment approval.

A GLP-1 medicine paid for by Part D at any point in 2026 rules you out of the Bridge for that year.

Ask Medicare and the person prescribing the medicine to check your details against the programme’s rules.

People who have type 2 diabetes follow the usual Part D route, rather than this programme, as of September 2026.

For your tissue-repair question, return to the exact medicine name and the insurer’s written answer about that use.

Check the visit and tests too.

Put the visit, lab work and medicine on separate lines when asking the insurer. Keep the answer for each part, including anything the plan asks you to do first.
